Ivan IV was crowned the first 'Tsar of All Russia' at age 16 and ruled for 37 years—transforming Russia from a medieval principality into a major state. He conquered the khanates of Kazan and Astrakhan, opened Siberia to Russian expansion, and established a centralized bureaucracy.

His second half was defined by paranoia and violence. After his first wife Anastasia died (he believed she was poisoned), he created the Oprichnina—a personal fiefdom ruled by terror, with a secret police (Oprichniki) who dressed in black, rode black horses, and carried severed dogs' heads as symbols. He massacred the entire city of Novgorod (60,000+ killed) based on suspected treason.

He killed his own son and heir Ivan Ivanovich in a fit of rage—beating him with an iron staff. The famous painting by Repin shows him cradling his dying son in horror. He died playing chess.
